Blue Cross and Blue Shield offers a variety of insurance choices and options throughout the United States. Depending on your state, Blue Cross and Blue Shield (BCBS) provides Health Maintenance Organization (HMO) plans, negotiated network provider (PPO) plans, combined PPO and Health Savings Account (HSA) plans, and Indemnity plans (non-network coverage). These plans (with regional changes) will be offered on either a group basis to employers for their employees or as individual and family plans or, in some regions, as both group and individual plans.
